Documents you'll need

For each person over the age of 16 named on your application, you must send us at least two forms of personal identification. One of these must show your current address, and at least one must contain your signature. One form of identification must also have a photograph.

Examples of photo identification: a passport, driving licence or travel pass.

Examples of non-photo identification: birth certificate, medical card, bank statement, utility bill or a letter from the Department for Work and Pensions (DWP) or benefit agencies.

If you do not own any photographic identification, you must provide a passport-size photograph of each person aged 16 or over, with their signature on the back.

We no longer accept documents by email or post.

We need your full address history for the last five years. We also need proof of your current address (for example a utility bill or bank statement), as well as your full address history.

You must provide this full five year history, otherwise we will not be able to process your application.

If you have children whose main home is with you, you will need to provide evidence for each child.

This evidence required is proof of Child Benefit (for example an award letter or a photo of the payment entering your bank account) as well as the child’s birth certificate.

You must provide confirmation of your right and that of anyone included in your application to live permanently in the United Kingdom.

If we require this confirmation, once you have submitted your application and identification a member of staff will contact you to arrange an eligibility test.

If you are pregnant, please provide medical proof of pregnancy and the date the baby is due. We will only accept a copy or photo of the MAT1B form provided by your midwife.

If you are in receipt of these benefits, you must send us proof. This could be a copy or photo of the award letter or the payment entering your bank account.

If you have access to a child but are not the main carer, please provide confirmation of your access arrangements, for example a court order, or solicitor’s letter.

If you are a current or former member of the HM Forces, please provide the relevant proof as follows:

• Confirmation of dates of service
• Confirmation with dates of residence in HM Regular Forces accommodation
• Confirmation that you are no longer entitled to occupy HM Regular Forces accommodation
• Details of illness / disability as a result of your service

Please provide confirmation of current value and outstanding mortgage. If your home is no longer suitable, please provide details / evidence to show why it is no longer suitable (for example a consultant’s letter).

Please attach details / proof of any action that you have taken to address unacceptable behaviour, for example repayment plans, support details.

You must send us details of any legal action taken against the landlord (for example, a note of disrepair).

Please attach evidence from Nottingham City Council to confirm your current status with regard to fostering and adoption.

If you are applying for Local Area Preference, please tell us the reason(s) from the list below and attach the relevant information:

• So your child / children can stay at their current school (you must provide a letter from the school confirming your child / children are registered and attends that school).
•  To provide or receive support for ill health. You will need to provide both of the following:

1. evidence of disability or carers benefits, or a letter from your doctor with details of the illness / disability and how it affects you or the person you are supporting
2. evidence of the address of the person providing or receiving support, for example a copy of a Council Tax or utility bill.

•  To provide or receive child care. You must provide evidence of:

1. work (for example a letter of confirmation from your employer, of employment and hours worked)
2. the address of the person providing or receiving support (for example a copy of a Council Tax or utility bill)

You must provide evidence that shows you meet one of the following criteria:

• Employment: A letter (on headed paper) from my employer confirming dates of employment and position held
• Disability benefits / retirement pension

Please attach any other relevant information which may support your application or help us to assess your housing need, for example support letters.
